Read up on different techniques and figure out what's best for you. Personally, simply focusing on breath control works real well for me. Others do mental images like a natural backdrop physical focus points like the flame of a candle. So long as you're actively relaxing your mind and suppressing external concerns you're doing the right thing.

I dont usually say something in my head. Yes, I pretty much observe the breath for long periods of time. If you have a mantra, say it out loud. If you are going to read a book, read it out loud, so that it becomes a task. I have recently realised normal people can't read outloud for shit, they hallucinate words

there are however meditation techniques were your object of focus is thought. With a little bit of practice you will soon notice random images flashing before your eyes and random voices in your head. But unlike daydreaming you try not to get drawn away by these thoughts but rather notice them and let them pass.
Don't get hung up on concentrating on the breath or on thought for that matter. There is an abundance of different techniques some of which might be more useful to you than others.

Look up headspace and start today, theres no specific time you need to meditate, some find it better in the morning, some before bed. The idea is that you learn the fundamentals and be able to apply them during the day.
