Just tall, 6'5". I know that isn't an excuse but it took me a while to get Squat form semi-decent.

How long do you rest in between work sets? You might want to take longer breaks, especially if your work sets look like 5-4-2.
If you're already doing that^, try having at least two days off instead of having just 1 rest day.
Exercise selection and diet will always depend on one's personal goals so i'd rather not touch on that.
