Can leg press 350kg, can't squat with 30kg. Whenever I squat/deadlift I get this pain in my lower back to the right of my spine somewhere in between L2/L5. Sometimes the pain lasts for a day, then it goes away, but I get the same pain when after a similar workout I try to tie my shoes. Once I was unable to stand/sit due to it being a constant stabbing pain in the same area so I was laying down for about 3 hours. Getting an MRI on the 11th, but was wondering if you could shed some more light. Some docs are guessing that it's a problem with the spinal disc, but the pain seems to be local.

Only on one occassion did it go to my leg and that was about three years ago in the midst of doing squats with heavier than normal weight for me (before these injuries started).

Could it be an issue with muscle tissue or is this positively a spinal disc sort of problem?

Being an ER doctor is interesting, I do procedures and it's the wild west. I rely on my wits and I'm a jack of all trades. I don't specialize and I rely on my skills that I've honed for well over a decade (not quite finished yet). As an ER doctor, I get to clock in and out and just focus on the interesting bits.

I'm gonna be honest, sometimes I could give 2 shits about who I'm helping, but I like the diagnosis. It's sort of like House, but less over the top crazy shit. I like doing a wide variety of procedures and diagnosing plenty of things. I mean, I could be a heart surgeon, sure, but hearts will probably get boring in 30 years. Who the fuck knows what I'll see at the ER! Granted it's a lot of illegals who go there because they have the beetus, but there's some really interesting shit there.

NSAIDs help, that's good, muscular that can be treated, it's the OTHER symptoms I'm worried about. Heavy lifts + weak core and back means you're loading up on your other muscles, which may shift form just slightly and compress something you're not supposed to. Like I said, see a doc, get some scans. Worst case scenario, you get a clean bill of health. But if there's something wrong, catch it early, we need imaging to figure stuff out.

Eh, pretty easy. Stimulants to fix ADD have relatively low side effects and really is a boon for people like you/lazy shits who cant focus. One of the few drugs that I can get behind that is overprescribed because it has relatively low sideeffects overall.

But you may need to see a shrink, it all depends on the doc. To make it seem like you dont have drug seeking behavior though, do the full PCP -> referral -> psychologist -> scrip

Bad news bro: If you think EM medicine is sexy? it's not. It's way less car crashes and broken bones. It's more fat people with the beetus, high blood pressure, old people who wait 9 months to see a doctor with a littany of problems, "I have abdominal pain" type shit. And it really depends. Some shifts nothing happens, people have the flu or an infection. Weekend shifts are usually drunk people who fell and broke their nose or overdose on drugs while drinking. OTHER times youll get like 10 people coding rushed to your ER. YOu really need to want to help people, not just deal with the sexy stuff, but it's always varied, keeps you on your toes.

Also charting. Be prepared to spend an hour after youre supposed to be on your way to your sweet, soft bed after your shift doing your charts, on a system from like 1993 IF YOURE LUCKY.

Join early, be active (don't be douchey), take a genuine interest, then when letter of reccs time comes for specialties, if you're not an asshole, you'll get a few good letters and have some demonstrative interest in both your academics and extracurriculars. if you don't clearly have nerve root pain (pain of neural origin i.e pain referring from your back down the line of a nerve) there is absolutely no indication to get scanned. the pathoanatomical view of non specific or 'somatic' low back pain is outdated.

yes, you probably have a disc bulge or some description but so do about 60% of pain free people your age, its irrelevant.

the pathomechanical view of somatic low back pain is the approach which has the most evidence supporting it. this approach links movement patterns and strategies to low back pain.

your best bet is seeing a good physiotherapist and sorting your shit out that way

People usually take a break to do something else. Do what you can to not burn out brah, you got a long way ahead of you. 4 years of med school, year 3 will destroy you, do not have a gf, do not have a social life. Go to the hospital and fucking sleep. It's only one year, deal with it. During surgery I had to be in the hospital at 4am and got off at 11pm. Have fun with that. You will be the bitch of the hospital but you will get better.

Don't be the one that passes out in cadaver lab, don't be that guy.

Also, don't fart in the ER. A chick did that, and we're pretty sure she didn't get her match of residency because of it.

Work hard, play hard Year 1 and 2, Buckle the fuck up for Year 3 and enjoy the tail end of year 4.

Also, of note, there are 3 and 4 year EM programs.

4 year = I am bored as shit, let me work already.

It takes about 3.5 years to be a good EM doctor in residency. You want to go to a good program that sees a lot of trauma (and you kinda have to hope people get hurt bad so you see them). So you can either feel green after 3 years or bored with 4. Also, consider doing a fellowship. My suggestion, do 3, do a fellowship (I'm EM and Ultrasound).

See you on the other side bro. It'll be literally the hardest thing you'll do in your life. I legit cried for waking up in third year. I thought about dropping out and if it was worth it a handful of times, but hang in there and work hard. You will come out the other side a new man.

How long do the fellowships usually take? Cause I wasn't sure if I wanted to do the 3 year with a medical toxicology fellowship or just do 4 years, or do 4 years and the fellowship. I kind of like your 3 year/fellowship idea, but what you said about being overwhelmed after the 3 years makes me kind of nervous to do that.
