It'd definitely lean towards it being something Rahm put there.
Which raises an interesting twist. I think this is the first indication we've had that she was directly communicating with the Black Tongues. None of the rumors about them even hint at it, which means it's a pretty well kept secret. Makes all the tattooing and castration a bit more understandable now: make it so they really have to want in to learn what's really been going on (titty bird has been helping them advance Pymary) and making it pretty hard for them to opt out.
