Remember, this is Harley at her lowest point. This is Harley trying to escape from everything she used to be, and failing. They confirm here >>83384672 that for all her attempts at doing crime and not caring about the consequences that she cares way too much to keep doing the things she does.
Genki Harley is pretty clearly who Harley wants to be, and she willingly views the world through Harleyvision as a defence mechanism. Liberman is showing Harley without those filters.

Depends on what you define as "crazy." Her mental breakdown was due to her making a slew of terrible decisions that hurt those around her while she was on her own. That's what the false Joker was supposed to be; he was her recognizing that she had been using him as an anchoring influence in her life. She knew from the beginning that the isolation wing belonged to her (as indicated by the messages with her fingerprints spelling out "I KNOW WHO," and her freakout upon her realization of that fact.) I would also go so far as to say that the ending in the park is ALSO false, considering the ease in which she is resolved of her sins by the girl she wronged and the instantaneous change in weather (it goes from sunny to a rainstorm in a split second.) Her committal was her relinquishing control of her life, but also relinquishing the guilt for her actions.
Her smile at the end was her returning to normal.
