Brevoort's excuse is that some writers and editors find it harder to remember than others, especially those working on stories involving multiple realities. I wouldn't be surprised if those people simply don't give a shit and stick with Earth-616 because they think it's fine the way it is and don't see the point in renaming it Prime Earth.
