Joker realizes he is insane and that Batman could save him but he is afraid because he is too scared of being abandoned by him in the process. He doesn't have the self esteem enough to think batman would truly care for him even if a big part of him wants to believe he can still be saved from that bad day. That there is still someone for him and that the world isn't complete darkness.
Batman realizes Joker will never gather the strenght to pull himself together and kills him, realizing that it is literally impossible to do anything else. He laughs, because the fact that Joker actually wants to be saved deep inside but won't allow himself is the ultimate failure for him and a huge irony. He will never be able to save his worst enemy, who deep down is the person that needs him the most
