In Marvel I've always felt it's implied that every other part of the US has its own thriving superhero community. Yost's Scarlet Spider went into Dallas' local team, for example. I wish there would be more comics going into that stuff but it's like wishing DC would go more into all their alt universes.
I imagine some small southern US towns would have like one superhero, and be similar to early Giffen Blue Beetle III.
