When she originally "doctored" Isabelle, she transferred her skin to her hand to replace her injury (but not heal it). She then went into the King Doctopus and swapped Isabelle's skin on her hand to the King's skin (the pink skin now on her hand) to actually tend to the wound. Once she did, she placed the "you're welcome" bandage on Isabelle's skin and swapped all skins to their rightful owners (the top three panels of this page).
Looks like he went back and flipped the thumb to the other side too.

It's as if Zack doesn't have like some sort of fleshed out bible on his charas as a foundation to begin his story early on + to proofread from as he writes the story along.
