I feel like the thought process behind Joker is that Sale was told to redo the Joker design for the Long Halloween and such, and was told to make the smile bigger. The editor kept sending it back to him, redraw after redraw, until Sale made it comically big. Then the editor approved and Sale just rolled with it.
