He spent more than a decade on Dimension Z (where time goes faster than in the regular world) with his son Ian until Ian was a teen and Steve didn’t age because of the serum.
At some point after he came back a Chinese villain named the Iron Nail took his super soldier serum and his age caught up with him.
Although Steve should be in his late 40's physically so doesn’t make much sense for him to be super old.

So the super soldier serum is basically magic rather than a chemical/biological thing, such that, as if it were a lame Twilight Zone episode, stuff ages super fast.
Except he was frozen, so there was no cellular change for deca ah screw it comic book writers are morons there is no defense of this.

I think the serum causing a drastic reduction in the aging process makes plenty of sense. Scientifically, well for comics anyway, it's a fairly sound concept where the chemical caused a change in his hormones that produce certain cells and the like. It's the same reason he can't get drunk and has a slight healing factor, obviously not to the extent of Wolverine or Deadpool.
Speaking of those two, the process they had implemented that allowed for their healing factor is quite similiar, honestly. So, there's precedent to the idea.
What's odd is that when the serum was removed that he aged so rapidly, but my guess is that without the chemical still in his body, his body went apeshit and it ruined him quickly.
It also could be like that creepy pasta about the part of your brain that makes you age. Since it was no longer being doped up, it realized 'OH SHIT. WE'RE SUPPOSED TO BE OLD.' and just caused rapid aging to what it believed it should be. Taking into account that his body went into a comatose hibernation during his time in the Iceberg.
