it does make sense. Mr Poopy Butthole's existence is as normal as Gearhead's existence in this universe. Perhaps he was just visiting the family at the time. The episode is set up to deliberately mislead the viewer into thinking that Mr Poopy Butthole is a parasite, to set up the joke that he isn't. It really isn't that implausible for him to exist in their universe - they take a vacation to hamster-in-butt-world for christ's sake.
