the hair got him an executive position which certainly pays more than working in Sector 7G. the dimoxinil was proven to work and would have returned the investment several times over since the episode proved it was the hair that made him successful. pretty foolish decision by Marge IMO

It wasn't covered by his insurance and it cost iirc $1000/month in 1990. 40 years ago a 12 month supply at $1000 was the equivalent of $21,000 in 2015. Even assuming Homer's salary could cover that, he'd still be at the mercy of the drug company that produced dimoxinil, as they could (and probably would) jack up the price - in fact, from 1990-2000 drug price inflation ran ahead of general inflation significantly, standing between 11% and 17% year on year. What that means is that by 2000, after 9 years at the lower rate, Homer would be paying something like $2500 a month to keep his hair - essential, since we know dimoxinil's effect diminishes in days or sooner once treatment is stopped, and he relied on it for his job. By 2015 drug price inflation had actually slowed, but still continues to be above general inflation by some way - he'd be paying at least $4000/month today, and that's all calculated at the lower end of the inflation scale, and assumes dimoxinil fell within that average.

Would you want to pay $48,000 annually from your household earnings to maintain a job that probably only brings in $80,000 before tax? Before you answer, remember the average US household earns only something like $50,000/yr, and there are far more below that line than above - it's just that the ones above earn that much more, including, for example, the largest concentration of billionaires in the world. That earning average has remained constant since at least the 1950s when general inflation is taken into account.

The truth is, even in 2015 Homer couldn't possibly afford to both pay his mortgage and pay for dimoxinil privately, unless he were earning significantly in excess of $100,000 before tax.

I think that eventually he would have been figured out. Coming across as cool and confident might have kept him in the position in the short term, but eventually they'd see him for what he was and realize that he didn't add anything. In fact Waylon had already figured him out, and the only reason Mr Burns did not listen to him was that he was enamoured by Homer and thought he was being jealous. If the Nuclear Plant was run with any competency (lol) Homer would eventually be fired/demoted sooner or later no matter what.

The real-life drug it's based on, Minoxidil, doesn't cost anything like as much (and never did). Even assuming your 6-month treatment is correct, that's still a minimum annual cost of $8000 in 2015; or at the higher end of that inflation scale, about $29,000 assuming 17% inflation since 1990 (which is slightly, but not much more, than real mean drug price inflation at the higher end of estimates for the same period). So assuming dimoxinil maintained that inflation, he's in the same boat.

According to salary.com (I know, I know), the median salary for power plant managers is around $132k, with a low end $10k below that and a high end $15k above.

If Homer is making $122k before tax, as the only taxpayer (head of household), he would pay 25% in 2015 in federal taxation; that amounts to $30,500, leaving him $91.5k from which he pays state taxes (harder to call, but he has an 88% chance of paying income tax based on the number of states levying them, given that we know from the movie he doesn't live in Alaska) which will likely be around 5 or 6 percent of his total income (including bear patrol tax) unless he lives in California, Hawaii, New York, or one of the other states with a higher than usual proportion of high earners. At a minimum though he's likely to pay that state tax (and we can in fact average that to 5.5% - $6710) so his earnings are already at something like $85k before we get to health insurance, mortgage, cars, college funds, average settlements of $68,000, food, clothing, and so on. Plus you know those suits ain't cheap, nor are Tom Landry hats or employee hammocks.
