It's not like cancer is a searing pain. It's usually a dull ache or strange sensation at first. Go to a doctor.
Excessive fapping does not typically lead to testicular pain. It can lead to pain but it is often featured in the shaft of the penis where the excessive friction is applied. The important question is where exactly the pain is located in the testicle. Is the pain global or are there certain spots that are more painful than others? Check if you can feel any kind of painful mass or clump. It does not necessarily indicate cancer, it can mean other things.

Do you wear boxers? Go commando? Do your balls move around a lot during physical activity? You might have testicular tortion. Only way to fix that is a painful procedure by your physician in which he will manually untwist your testicles.
