How do I get rid of fetishes?
I have some absolutely degenerate extreme ones that I could never tell anybody about (they luckily only involve harm towards myself, never others, only adults as well)
I'm at the point where I can only jack off to thoughts about them, and anything that I encounter in my life that slightly reminds me of them, turns me on to an insane level.
They're all terrible fetishes and I hate myself for being as miserable. They made me end up in the ER twice already (although I could talk myself out of it) and not much permanent damage was done.
Is there anything I can do about this? I'm desperate for any help

The first step is deciding if you really need to get rid of them. If you to any degree feel that you don't, it's going to be more difficult.

I suppose it'd be good to get rid of them because ~see above. It obviously turns me on though and it's satisfying to get off. So I guess my brain connects positivity to it, but I'm not retarded I need this to be cut out of my life.

I'm in the same boat as OP. I had a girlfriend once and she cheated on me but I decided that I'd still try to make things work. Then she dumped me because I couldn't completely forgive her and I'd get angry easily.
Then at one point after she dumped me, I saw her facebook profile was with another guy and I figured "Hey, fuck you I'm not going to feel sad about seeing you with other guys. In fact I'm gonna make it give me pleasure. I'll jerk off to the thought of her fucking other dudes. A pornstar fucks other dudes and I don't hate them. That's all you are to me, a pornstar."

Honestly, not much shocks me these days. Anyway, practice cognitive behavioral therapy. Set up goals you want to do (such as avoid using your fetish perhaps five times in a row when you get the urge, increase the number as you go), reward yourself when you succeed (obviously do not reward yourself with indulging in your fetishes). You should also get to the bottom as to why you began practicing these fetishes and deal with them, if there is anything in particular that is troubling you that has perhaps resulted in these kinks. Try and fix the problem if you can, but just knowing why, accepting it and forgiving yourself to why it happened can mean a lot for the mentality.
