Squeeze some fresh lemon in ice water. And parsley helps too.
Brushing , flossing, tongue scraper and mouth wash TWICE A DAY, then gum after you eat.
Every 6 months per routine at a dentist
Unfortunately some people are genetically predisposed to stink.
Does your mouth get dry?
I take several medications and my mouth gets dry at times. I brush twice daily and brush my tongue with a tongue scraper, that seems to do the trick. I also make sure I drink plenty of water.
Try chlorophyll pills, or just eat a bunch of parsley.

I forget what they are called but a main source of some really persistent bad breath is because of tonsils. essentially gunk gets trapped in your tonsil cavities and sits there making little pearls of rotten stink. these nuggets will end up secreting small chunks and giving off foul odours in your breath. to make things worse normal brushing flossing and rinsing (even tongue scraping won't work).
if you find that rigourusly following peoples advice doesn't solve the problem after several months consider getting your tonsils checked for this problem at your dentist.
if they confirm it then consider having your tonsils removed. super easy fast and cheap procedure.

Water.... stay hydrated... lemon or cucumber water is great
Mouth wash... any...
Ora-brush is good (you can see the brownish white stuff that comes off when you scrape your tongue) and your breath doesn't smell nearly as bad
Mints (get some excel mints and put one in your mouth before an intimate conversation... just don't eat the whole pack in a day)
