How do I break myself from routine?
I should specify.
Everyday is just the same, I wake up forcefully, commute, go to work, get back home, sleep due to lack of it at night (and I don't sleep at night because of it), take an extensive shower, because I feel dirtier than what I am, masturbate, then sleep late.
Rinse and repeat.
All of this, combined with my male impotence, failed ambitions, cynical outlook, have made me miserable.

There's nothing inherently wrong with routine. You're just sad.
So fuck off honestly

How do I fuck off?
Help me help you, by telling me how.
Just fix you eating and sleeping habits, make friends, get a girlfriend, a hobbie, you have probably thought about this. You know the solution, it's your choice to follow through with it or stay miserable.

But how are the terms of such actions, do I change my habits slowly but surely, or should a dramatic point suffice?

First off: sleep early tonight, please, make it an habit of yours. This is probably the best thing you can do right now.
