What do I need to rent a car in the US besides a license?
a car

A credit card. Most rental agencies won't even take a debit card.
That's really about it.

What about insurance? Do they include it?

Yes, the cars are insured.
But don't crash a rental car. It would be obnoxiously expensive.
Some companies will offer a coverage option that's really expensive and pretty much just a way to rip off paranoid people.
Basically, deny all of the additional services they offer you, and don't crash the rental. And fill it up before you drop it off, otherwise they'll bill you like $10/gallon of gas to fill it up themselves.
