How do I stop being so lazy to the point of it being self-destructive?

Stop fapping or playing video games when you're bored. Exercise at home or ride a bike in case you don't have friends to hang out with. Also stop eating so damn much.

My problems are that I'm holding 2 D's and an F in the classes I'm taking because I can't get my shitty mentality from when I was a NEET a while back.
It's bad to the point where I don't even have my drivers permit, because I keep failing the test by 5 to 6 points. I just want to know how to self motivate.

Start with small things. Keep a calendar, don't get carried away (harder than it sounds). You're fucked on the classes. Study your driver's handbook every day or find ways around driving (I'm 28, fully employed, living a normative life, I've never had a license). Start exercising daily, even if it is just walking around the block a few times each morning. Get on a regular sleep schedule (try for 6-8 hours a night). Start slowly trimming back on vices.
Doing enjoyable activities because you can't work yourself to do anything good for yourself is not quite "laziness". Laziness is like, a dog basking in the sun. Maybe you should reframe your idea of what is going on here.
