It's certainly wrong to assume this of all women. That would require them to be read minds. But telepathy is not the only way to discern someone's intentions. If you've somehow made them clear, or failed to hide them and she picked up on that, then she knows what she knows.

Not necessarily, especially if it hasn't been very long. But the longer it goes on, the greater the likelihood that it is intentional.
