I like most of the spell effects, but not all of them, and I'd like to ask a few questions.
I get that you're not necessarily fucking people for the mechanical bonuses, but there's no way to take advantage of that unless you enjoy quickies before battles. It should be at least one hour.
You said they'd leak milk from their breasts for 1d7 days - that'd be a bit hard to roll, though I can understand why you'd want to keep it within a week. Might want to just bump it up to a 1d8 instead. Is the breast growth temporary as well?
If you wanted to be very lewd, you could pair Keep Watch with some vague lust effect.
Killing off a character, even with a new one being made immediately after, is a bit much. And it's a new race as well as celestial? That might be troublesome if they were a half-orc before and became human.
I'd personally cut down the effects with a 24 hour limit to 12, but that's really just being picky about time. Bestow Grace of the Champion is a pretty powerful spell.

And for the reincarnation thing, why not replace that with something along the lines of giving a certain amount of experience (so there's a chance of leveling up), or maybe gets to pick a bonus feat? Not really sure what to suggest, so I'm just throwing out ideas.
You could try roll20? That'd probably mean using text, but online playing isn't that bad, and with a smaller group you could probably get away with DMPCing. Hell, I might even offer to join you if you found a nice enough group. It'd be nice to play a female barbarian for once.

I think of the numbers of times you get to use this effect should be based off the paladins cha or con scores.

Personally, I'd say use con. Just so that it gets used for something, since it's kind of a useless stat, to be honest. Cha might be mechanically better though, and it works better with the idea that the sex is divine magic like paladins use.

The one cup thing isn't bad - I actually like it a good bit - but another idea would be to have it go back down two cup sizes over time. It'd be a fun excuse to regularly screw to keep them big, and it'd keep it from getting excessive after the fifteenth roll. Well, unless you like that.
The cum thirst effect seems better than straight up lust, yeah. Especially since you're giving a better chance of resisting it as long as they pass their will save - and depending on how often they fucked, they could get a bonus for that with a different sex effect, so it's not
unfair, especially for an intentionally lewd character.
Shifting the target towards the paladins alignment is a great idea. Goes along well with the 'fucking them to goodness' thing.
And yeah, that holy tattoo thing is really clever! A bit more than just a bonus or a buff. Maybe you could add some sort of anti-succubus effects? It'd be fun to have these lewd paladins be sort of a counter to traditional sexual devils and such.

I'd expect magical realm in my erpg, and even want it. As long as people are ok with the other's magical realms they can all pop up. I've had a bit of it pop up irl and it can be a bit weird, so def prefer it online. I don't think you'd need more time to do the game, your sessions just wouldn't make much 'progress' but that's not really what the games are for.
It does take time for groups to form though. Especially online. I'd prefer a smaller group anyways maybe one dm and two players. that leaves room for more 'bad endings' and easier erping

A small group does make sense - erpgs should be a bit private anyway, and if people are going to be indulging in magical realm, you should make sure everyone's okay with the other's fetishes, or they at least won't get in the way of each other. And a smaller group allows a DM to focus on players a bit more personally, since it's probably not going to be a game too strongly based around huge combat or drama, but more interpersonal interaction and...well, sex. If everyone knows what they're getting into, it should be fine.
I don't really like bad ends, though. Dying in battle is one thing, but the stuff that can go horribly wrong in something erotic is another.

Bad ends can be fun if the DM is willing to make them made for erotic pleasure rather than brutal rape. A good DM will work with your fetishes, or other players. Besides in erpgs Bad Ends are basically how your character never dies. its like resurrections, except not having to wait for the other players to beat the dungeon and find a healer.
