My usual process is to unfold the diaper, and then gently bend it together horizontally. So that the leg guards on either side are touching, pretty much. Then, when you do the tapes, start with the bottom pair and have them both pointing upwards at a 45 degree angle. Do the top pair second, and have them angled downwards. If you still find that the diaper's a bit loose around the waist, then you can sit up and pull the diaper tight before doing the tapes.
As for having it hold up over time, the best thing you can do it just make sure the tapes are tight. If it's loose because it's sagging, then there's not much you can do outside of wearing something like a onesie.
About the only other tip I can think of is to make sure the area you're taping is flat. If there are any folds or creases under the tape, then they'll probably end up being pulled loose when you start moving around.
