If a magical girl manages to survive for a thousand years without succumbing to despair, would she still look the same way she did on day one, or would she be a literal skeleton?
How old was Mami when she contracted? She looks more grown up than the other girls, but she's barely a year older than the rest of them and she was a magical girl for a long time. So did she age as a magical girl, or was she already well developed when she contracted.

Urobuchi addressed this in a QA with Hanokage. If a magical girl's body is destroyed beyond repair, her soul gem will remain intact and thus "alive" but they will quickly panic and despair.

I wish I could find the QA, but I believe once they realize they have forever lost their sense and all, they will panic and despair. If their body is destroyed, they wouldn't necessarily feel it as their is nothing left to feel. To your second part, this is most likely the case.

Butcher also mentioned that if you don't know the secret of the soul gem, a magical girl can despair from getting an arm cut off or something because they believe they're dying. Had Mami not had her gem on her head, she would have become a witch for this reason.

It wasn't healthier. Homura was very broken, to the point of coerce Yuma to contract, with her wish being a catalyst to make Homura pregnant of someone who would be exactly like Madoka.
