Subtitles were never intended to be translated script. There are standards on their speed and length that don't distract the viewer from the original work (anime fansub doesn't give a fuck about them, though).

Fan translation in general should always include honorifics and respect Eastern name order, especially when the surname is used instead of first name. Its usually a big thing if or when characters switch to less formal/more intimate name referral for various reasons.

Blame gg and Commie. Their cancer of Americanizing instead of translating specifically to the general anime/manga/etc crowd have spread far in anime fansub community. Thank God the majority of manga, LN, and VN groups haven't been tainted by them.
