I don't think he escaped. He's the guy who made a whole village kill each other and did several other massive shits, no way there weren't at least some policemen guarding his room. To me the end is more like, either Johan is dead, then it'd prove Tenma's point that all lives are equals since he tried to save him yet failed, or a metaphor that implies there's no monster anymore. In any of those cases, that's a great ending
I always thought he killed himself by jumping out of the hospital window at the end, since he's lost all purpose and would probably have to serve life in prison
