Here's how you do it:
Vampirism is a disease that can be passed by mingling fluids, like an STD.
People who are vampires have their cell division cycle loop infinitely, and regenerate damage incredibly fast (like 30 times faster), and can regrow lost limbs, including their brain. Brain impulses are reset to the state of the disease's complete takeover every 2 weeks, so Vampires can never age physically OR mentally, and cannot form long-term memories. Some vampires aren't even aware they are vampires.
This is especially problematic for vampires who didn't learn they were vampires before the change completed, as they can have their brain impulses reset and not know how to feed.
Lack of blood causes an irreversible degradation of the genetic code of a vampire.
They become more bestial, losing human traits like memories, higher thought, impulse control, and even begin to look like horrible monsters as the degradation progresses.
This damage is permanent, and even lost body parts or destroyed brains will not regain their starting condition. The only upshot to this is that they will retain any memories or knowledge gained while "starving".
Fully degraded vampires are vicious and voracious killers. they hunt with great stealth and cunning, and kill far more than they need to survive. They are stronger and faster than more "human" vampires, and are nigh-impossible to kill unless they are drained of blood, incinerated, or otherwise destroyed beyond the point of having anything left to regenerate from.
A vampire who drains a degraded vampire will become "blood drunk" for several days and regain the ability to age and imprint new memories on their "default" brain.
A degraded vampire who drains a human vampire becomes permanently stronger, faster, and more cunning, and can regain some intelligence.
