Well, I never saw this as anything sexual, guess it is for some people, but it still doesn't really matter to me. Also it's an action manga to some extent, nothing wrong with having some guro in it. Randomly letting a girl wear diapers which has zero relevance for the plot is something entirely else, though. As for the pissing, that was also unnecessary but if I remember correctly it was just one short scene and at least instantly made clear that they would need to completely take care of Niu, so it had at least SOME relevance for the situation at hand.

The Elfen Lied manga doesn't try to be deep by any means compared to the anime, but the point of Kouta owning the inn and a bunch of girls moving in with him was that he was taking care of and giving shelter to people who had issues that society would shun them for. Lucy and Nana were diclonii, Yuka was incestuous, Mayu was a runaway rape victim, and Nozomi was incontinent and diaper-dependent. Your reaction to her pretty much proves the point the manga was making.
