What they've been hammering into us is that All Might has been on the verge of death for 6 years, and OFA is probably the only thing keeping him going even as it actively strains his body's injuries as he uses it. Even if All Might passed OFA onto Deku completely and retired from heroing, his body would probably have succumbed to the whole "missing most of his internal organs" thing once OFA was gone.
I don't think AFO will be killed, however, he will likely suffer a even more serious injury then last time, and gets hooked up to a golden throne type deal.

Mandalay and Tiger were pretty actives and in the field, point for them.
Aizawa and Blood King basically played Dabi's game by staying in the base to protect the students for a single clone. Aizawa shortly got out and met Deku but then went back inside with Kouta. Dunno why one of them couldn't have gone to lend an hand to the students fighting in the forest. You might say that they had to protect the guys at the camp, but the only villain was Dabi (not even really him, his clone), and it just seems weird to me that they refuse to let one part of the class help and let the others sort their shit out themselves.
