I can only think of an extremely small number of shows that are improved by a dub. cowboy bebop, space dandy, that's pretty much it. a lot of times the dub doesn't even make sense tonally. like samurai champloo. the dub is fine, but why would you want a bunch of samurai to be speaking english?

Really depends on how much the mangaka cares about the animation (ie. how much free time they can squeeze from their work on manga). If they have the dedication, they can basically nitpick about anything and make production do as they want because the position of original creator (gensakusha) is really strong in Japan. That being said, if the communication between author, magazine editors and anime staff is really bad, then something like Shirokuma Cafe can happen.
Lots mangaka both suggest people for roles and come to listen to the auditions. Sorachi Hideaki apparently said that he picked Sugita for Gintoki from the audition tapes because he was the only one who actually horsed around.
Lot of them also come to the anime recordings every week just to hear their characters coming to life. Shaft might the most friendly studio for creators because several of them (eg. Aoki Ume, Kobayashi Jin, Nakamura Hikaru) have come to the shows' web radios and told how much direct input Shinbo asked from them.

The Persona game dubs were pretty good except for adding senpai. Those weren't bound to mouth movements like anime dubs are though.
People that speak both languages fluently tend to agree that dub and sub voice acting is equally hammy with the occasional standout performance on both sides.
If you want to listen to a language you don't understand and pretend it's well-acted go right ahead, but don't piss on my leg and say it's raining.
Subs capture Holo's playful side better and are perhaps more appropriate to the body she's inhabiting in human form. Dubs capture her mature and haughty side better.

Honestly, it just comes down to preference. I tend to watch dubs if they are available because I like to focus my eyes on the animation and my ears on the words, as it were. Granted, in my opinion there are some shows where the original Japanese fits better (for example, Kill la Kill), but really I'd say most dubs post-DBZ are actually quite good (Durarara and Sword Art Online come to mind as specific examples).

In regards to "how the creator intended it to be," there are arguments for both subs and dubs. In Japan, the show is in Japanese, and therefore that is how it was "intended to be" and so subs are the way to go. But if you think about the fact that the Japanese are not reading subs, but simply listening, then maybe that's the pure way to do it so you don't read but instead listen as they would in Japan.
