Author Rumiko Takahashi did cover what happens with pregnant people who transform back - miscarriage.
There was a village that used to mate with animals it threw in the cursed spring of the drowned girl. However they had to use a special magical item called the Pail of Preservation that made the curse permanent before getting them pregnant so that they wouldn't switch back. Ranma can therefore be safely impregnated once the water from the Pail of Preservation is poured on her. Otherwise an accidental reversal would presumably result in a miscarriage.
By the way, the fact Rumiko came up with the Pail of Preservation to explain how animals turned into women could be impregnated and give birth to children proves that she thought about and came up with a solution on how people that fell in that spring could become pregnant despite her claims not to think about whether Ranma could be impregnated. Rumiko's famous irritated response that she doesn't think of such things and neither should YOU was made because she was at a convention panel. That question put her on the spot and irritated her.
Picture: There was a food product line for Takahashi's various anime shows called "Rumiko Takahashi's Taste of Home". I'd say to just read it. After the third season most anime episodes are anime original, with only a few adaptations spread around them, but the anime writers let things get stale too fast - for example, Ranma never uses the techniques that he learns later on in the show during the filler episodes. They constantly reuse stories like "apparently powerful opponent is just using a trick", even while the later manga stories, some of them adapted by the anime, feature characters using actual ki blasts and such.
There are a few good anime episodes in the middle of it all, and they did improve some things (like Dr.Tofu's appearances in the final seasons, while he was just completely forgotten in the manga, while he even gets battle scenes in the anime), but I don't think it's necessarily worth watching.
