Sopranos had good scenes. Shame anime hasn't evolved to that level of maturity yet. I suppose often the nukes are blamed. But Japan had tentacle porn long before
For a single scene the most important things for me are shot composition(cinematography) and music.
I doubt anything will ever dethrone the train ride from Spirited Away as my favorite scene from an anime.
Good music and visually pleasing. And/Or emotionally powerful.

Usually a 10/10 scene for me has some sort of impact, make the viewer angry, sad, happy, whatever and couple it with a great soundtrack. But everything has to be tied together in a good way, realizations shown and maybe a memory or two depending on what happened. These kinda things are usually at the end of a movie or something though.

Sometimes, shock value will make things memorable (the first episode of Ga Rei Zero, Madoka's 3rd episode, the end of Gakkou Gurashi's 1st episode).

Buildup, music, and visual impact. It can be one, two, or all three together. For example, in Sakamichi no Apollon, the scene at the rooftop in episode 10 with the electric guitar music was very memorable for me simply because of the music and buildup, the visuals weren't anything special. On the other hand, pretty much every episode of Uchouten Kazoku has a memorable scene because of its visuals. The whole few last episodes were also memorable because of the buildup.

No, I noticed that theme. The multiple monologues about the town were clear enough. But see, there's my complaint with the series in general. Why did it have to be perfectly happy or completely crushing tragedy? My first time through after Ushio's field scene, I thought that was a perfect track for ending the story. The city still becomes a place he can love, even if Nagisa's not there. I thought it was nice that he didn't "need" Nagisa anymore, and had created his own happiness in the town he hated. Someone wasn't saving him anymore. He was growing as a person. I would've ended it on that note.

Oh, I'm not saying a bittersweet ending would've been bad, I'd actually prefer it as well. The problem is that making it so would make the both the orbs of light and the other world meaningless in the context of the story. To change the story, you'd have either needed to use them to magically cure Ushio at the end or remove them in their entirety, with the former being an extremely underwhelming execution of the established plot elements and foreshadowing and the latter damaging the story as a whole due due to how it would change the setting and central themes. At that point, you'd be better off just rewriting Clannad in its entirety from the very beginning.

For me, the entire FMA:B OP1 is what makes the show.
The whole OP reminds the viewers of the past that the brothers have gone through and why they're on their journey. This way, I get to feel the motivation that put the Elric brothers on the road in the first place each and every episode (of the first cour anyway).
Coupled with great animation and even better music, it's the #1 OPOAT.
