Second way is, you know the target audience it will appeal to and try to make the protag as most conforming to that target audience. So an otaku show will have a virgin, neet, techie character. And that will be all to them. No negative aspect. No personal backstory thats it.
If you haven't noticed, in both cases, the onus is on the creator as opposed to the audience. The fact that so many can see themselves in Naruto or Hachiman does not mean by their very nature the character is a self-insert.
How many people view Shinji in a positive light for him to genuinely be a self-insert. Very few. A self-insert character is one made with the audience in mind to project onto. They don't need a storied background or genuine characteristics because the audience will place theirs instead.

Rei is an artificial human being so in a narrative sense it's fair she'd be so distant and unknowable. But even then, she's not a blank slate as illustrated by a post on /mu/ I can't find that ask what music does she listen to.
The first reply was she doesn't listen to music, which many agreed to. This is a character trait not many can relate to yet it fits Rei given what we know and don't know about her. Hope this makes sense.

This was for the sake of longevity. Ichigo used to be a redhead japanese punk no one would fuck with. He saved Rukia because she did him a solid and he had to repay her. Emphatizing with a character who is not like you is alien to them. They project self-insertion onto Japanese otaku even though they watch stuff like Yuru Yuri with no male characters in them.
Self-insertfags also can't interpret characters beyond the most basic, superficial level which leads them to think that characters are all the same (e.g. every male lead of a harem show is exactly the same).
