It seems that Kaneki was slowly losing functionality in his own body by replacing lost organs and limbs with kagune constructs. The last nail in the coffin was when Arima ran him through with Owl. Also Arima was wearing another quinque according to spoilers. One similar to Arata called I-human, which is what saved him from Kaneki's final attack. Arima compliments him and says that it was still too soon for him. Shortly after Kaneki lost control and was consumed by his own kagune which grew to an immense size, but Eto swallowed him and powered up. It's not known if she ate him, or if she just brought him in and hooked him up to her kagune to leech off him. Her kakuja form became dark.
Also, Ayato died while Touka lost an arm.

Arima is avoiding Kaneki's attacks and attacking back. And he says something that manages to snap him back to sanity.

I think that Kaneki planned the whole thing with Ayato to begin with. After all, he went back up to save them with no indication of them being there, and he planned to die to Arima. So someone has to escort Hinami out of there.
He just didn't expect Touka & co to be there too. And I guess Ayato didn't know that Kaneki was going to break her out first.

Ayato had no clue who was opening cells or who sounded the alarm before they did. He may be playing dumb but it wouldn't make any sense why he would. Kaneki even had to even make sure that they were escaping the same way he planned for Hinami.
