It's a sol mix of his work and personal life
Toi was a hardcore Otaku and only ever wanted to make anime. His parents didn't want him to waste his genius brain on trash so they pushed him to go to medical school which he graduated at the young age of 22 specializing in gynecology. After finishing med school he moves in with his 24 year old sister Mei. In spite of her being a total normie and him being a weird otaku they have always been close.
His first day as a gynecologist didn't go quite as expected. Most of his patients aren't as young or attractive as in his favorite harem anime and the smells are more putrid and foul than anything he has ever smelled in his life.
When not at work he spends all day watching anime, reading manga, and shit posting online.
After a while his sister tries to get him to break away from that life style. At first he is reluctant but then he decides to take her up on her offer an go out with her and her friends from work.
Over time he slowly becomes more social and starts to become friends with his sister's friends.
One night at a club her meets this girl yada yada yada they have a drunken one night stand. Only he thinks she actually loved him when he finds out it was a one time thing he falls into a depression and regresses. Soon something far worse happens
While at work he notices his sister is one of his patients. Even someone depraved as him doesn't want to examine his own sister but he can tell by the look on her face she is really concerned about something. So he sucks it up, straps on his gloves and gets up in there. He notices a lump. He takes a scraping and sends it off to a lab.
When he gets the results back he holds her and they both cry. Right before she goes into chemo he makes a promise that he will marry a good girl and make her a proud aunt.
He eventually marries a cute biology professor.
The last scene is of him, his wife, his twins, and his sister having a nice family outing at the park.

MC, a philosophy major at Sussex University joins the Libertine guild, only to be raided by the police during its first meeting. MC and several other members managed to escape through the aid of an unknown hacker. After escaping, MC and these members are given the option of receiving enlightenment. While the others reject out of fear, MC accepts and is drawn into Libertatum, now fighting against the Labor government and ultimately take down Anita Louis.
MC is a young luxury hotel manager who has recently gotten engaged to the girl of his dreams. Her rich father initially objected, but approved once MC demonstrated his ability to take care of her by buying them a sprawling home in Tokyo with his new salary.
Due to Yakuza-linked shenanigans at the bank, MC is duped into signing the wrong documents, and he inadvertently becomes the new owner of a love hotel in Shibuya. The Tribe capture most of current modern people, which The Tribe dubbed them as 'Yamato people', and turn them into slaves. But some of them managed to escape from getting capture and form a resistance called "Children of Amaterasu".
After the mainland rebellion was over, The Ainu and The Ryukyuan declared independence and claim Hokkaido and Ryukyu islands as territory of their own respectively. They refused to join The Tribe because the resentment to some of The Tribe member for past invasion. Both of them are ruled by young High Priestess Regent, chosen to lead their own people according to each native's belief.
Our MC is the last descendant of Aterui from the Emishi people. He did not join the rebellion and prefer to remain at his home (somewhere in Tohoku region) to wait the current event passes and continue on his comfortable life. However he got tangled deep into this mess as he stumbled upon Ainu High Priestess Regent herself, whom just lost the war with the Koshibito and tried to escape from the rest of her pursuer.

MC confronts the main baddie man and gets destroyed. But thanks to a deus ex machina his green crystal grants him great power and MC-kun absolutely destroys main baddie. The power soon backfires, black mist oozes out of the crystal and consumes MC-kun. Companion-chan can't save him and is forced to escape as massive amounts of energy erupts from MC-kun and the surroundings are overtaken by black mist/goo.
5 year time skip happens, Big Baddie is actually alive and meets up with Companion-chan, who all this time hasn't been able to get back to MC-kun, to offer her help.
Companion-chan is actually the real MC of the story and the rest of the story is about saving MC-kun with the help of her rag tag group of friends and Big Baddie.
In the climax, thanks to powers granted by Companion-chan's pink crystal, MC-kun is freed from the black shit, but it takes a new self contained form. The weakened MC-kun looks from afar as Companion-chan, leading her rag tag group of friends, gives an inspiring speech about power of friendship or some shit, and MC-kun realizes that Companion-chan has become the spitting image of his dream.
They then defeat the evil...but what happened to the capital? see you in the sequel!

I-i actually have a series i'd love to be able to release either in paper or animation. I still have very little plotwise but I have drawn out the characters. The very basic that I have planned is that they would be "Scavengers" in a post-apocalyptic but recovering world that had to resort to living underground due to the air conditions of the surface. Scavengers are people from organized groups/gangs that people pay to bring stuff from the upper world (from water, metal, to a photo in a chest buried next to a tree in x place) since normal people still can't go to the surface.
The story would be mostly SoL-ish at the beginning and then reveal a bit of a larger picture by the end and always up beat and chill even with the somewhat somber setting. Ideally it would run 25 episodes.

Meanwhile, the old master is training a new student, but is still haunted by the MC's failure. The new student of course is the true Chosen One, and he sets out to defeat evil and spread justice. In his travels, he comes across the MC. They dislike eachother, and the Chosen One dislikes the Lord, but they set aside their differences for the greater good.
The whole story is about how the Chosen One, the Lord, the wise old master, and various other factions react to basically having a renegade 1.0 version of the Chosen One on the loose; and whether or not the MC will decide to become the hero he could've been if he hadn't gone on his quest for revenge. Perhaps the MC crosses a line and the Chosen One decides to fight him, perhaps he comes to respect his Lord for whatever good qualities he has, perhaps the old master tries to convince the MC to return home, perhaps the Lord or some villain wants to use the MC for his own ends.
Basically, it's a story of the hero's renegade anti-heroic rival from the anti-hero's POV. I don't really have too much of anything concrete, so maybe the MC has a different reason to be dismissed by his master--the dead love interest is a pretty cliché anti-hero backstory, I'll admit--and I'm not sure if the Lord should turn out to be evil or just somewhat scummy and antagonistic toward the Chosen One.
