The only MMO anime I can think of that came out recently were Sword art and Log Horizon though. There's fucking tons of fantasy coming out, but none of it is like, elves, witches, magical swords, and dwarves.

There's multiple reasons for that, even putting aside the old art style we'll never get back.
1. Light novels largely dictate trends in Japan when it comes to this shit and they're basically YA works of fan fiction level of quality
2. medieval fantasy is not a default fantasy setting in Japan like it is in the west so it's not nearly as widespread
3. all their escapism outputs; manga, video games, etc, are closely tied together and are tapping an overlapping fanbase so all of the above spills over.
Shingeki no Bahamut: Genesis was good modern western style fantasy. It didn't have elves or dwarves in it, but it did have demons and liches and shit like that.
It also had noses.

My issue with modern fantasy anime is that the writers seem to be missing the point of why fantasy tropes exist and including them solely because 'well, fantasy has to have elves and goblins, so...'
It's like their experience with fantasy is only online MMOs- they know the aesthetics of fantasy, but not the themes- their fantasy is a copy of a copy, aping the appearance of fantasy and totally missing the point.
Slayers and Konosuba are a good comparison, I think. They're both comedy full of goofy things with occasional drama, and a sort of sloppy 'Something something demon lord and monsters,' but the world of Slayers is deeper, and there's a sense of discovery and an existing, living world. Whereas Konosuba is just 'well, it has fantasy stuff sort of all thrown together to enable jokes.' Slayers had plenty of stupid comedy, but it didn't have to sacrifice fantasy and wonder to do so- the comedy was part of the world, but the world wasn't part of the comedy.
