Boku is a normal male pronoun, I guess any girl that uses it would come off as odd if they're not trying to be tomboyish

Both, but mainly males. It seems that this pronoun implies child-like craziness.

I noticed that whenever an adult character uses boku instead of ore or watashi, chances are high that he is mentally unstable.

It's more that boku is used to demonstrate a degree perceiving oneself as a immature or, in these cases more likely, a wee child (or wanting to foster that image) and creepy child/manchild is a well-tread cliche for bad guys

Sometimes characters without any os these traits use boku for some reason. Kiritsugu for instance.

I'm pretty sure Kiritsugu's use of it is an indicator of how psychologically stunted he is. He got obsessed with something at a young age and continues to pursue it despite the damage it does to himself and the people around him.

Hell he wanted to wish on a magic lamp for the end of all suffering, or whatever, and when confronted with any counter-arguments he either shuts them out entirely or pitches a hysterical fit. If that's not immature I'm not sure what is.

In certain regions, it was normal for women to use 'ore' or 'ora' to refer to themselves. Not sure if it's done nowadays, though.

"Boku" is what young boys tend to use, and when adult men use it it generally gives off vibes of humility and innocence. This can be played for creepiness (it's most common use, think of a slimy used car salesman or psycho manchild) or used for genuinely good characters to make them sound either more naive or servile.

When girls use it, it's generally a sign of precociousness. I don't know about adult women, probably the above villain thing.

Completely deliberate. Using ore when talking to a superior at your workplace would be more or less the same as showing up with a huge YOLO tattoo in your neck. It's literally ghetto tier speech.
