The anime takes all of the negative sides of it's characters and cranks them up to 11. In the VN, Sora is still in love with Haru, but will accept it and give her aprooval if he dates someone else. While partially true, I don't think that if Manami were the imouto, she would act the same way. Kirino's imouto, pre-Sakurai, post sakurai, and post vol 11 (or whichever vol is the Manami-Kirinio summit) show how I think her being his sister impacts her character.
I guess a smarter way for me to put it would be, her being his sister makes certain things make sense, like her idolizing him and being frustrated when he changes, rather than just moving on.

I think most of that would still make sense if Kirino was a family friend and Manami was the imouto. You don't have to be a sibling for somebody to idolise you, obsess over you and hold you to unrealistic standards.
