That's usually the case in a harem or for a side romance, not in something that is primarily a romance.
In which case, it is clear what the couples are a few chapters in if not the first chapter. How is that any different from an American romance movie/novel where the entire story will focus on the main pair?

On top of what others have said, I think that it's also worth noting that Japan's male-oriented romance fiction and the west's housewife-oriented romance fiction are two entirely different genres. A few differences:
Male-oriented romance fiction tends to be about wacky antics and cute girls.
In male-oriented romance fiction, the protagonist is usually a problem solver for his love interests. In female-oriented, the protagonist is a problem causer.
Following that, problems in male-oriented tend to have an external source, to which the relationship plays a part in curing.
tl;dr: Housewives are horrible pieces of shit who want to fill their partner's lives with suffering for the sake of their egos. Men want good times and stability.

Kinda. There's only one problem with them, and that's the obsession with "she's never been interested in another man before me, and she'll never be interested in another man after me, either", sometimes even going so far as to make sure that the losers never get on with their lives. I say "kinda" because that one problem is easy to overlook for people who wouldn't hold that expectation, due to the fact that the rest of the content (99% of it) is all about cute girls and fun. It's not the foundation on which the story is built. The prior and post-story love lives of the characters aren't always specified, either, so this is only in some works.
