Check the panels in the last chapters which show post-coma Satoru wandering the town and constantly crossing ways with Airi (usually shown somewhere in the crowd/background).
The author made them a fated couple from the very beginning, and used those panels to pass the message - no matter what path Satoru takes, he will eventually be given his fateful meeting with Airi, and from there, it's up to him.
Note that the very reason he jumped in the first place was because his mom was murdered, Airi was nearly murdered, and he was about to get put away, leaving Airi at the mercy of the murderer.
Sure, Satoru did humanely care for Kayo, but she was means to an end for him - a way to save his mom and Airi.
On his time-travel of sorts, he's shown to have gained a pair of married friends, a success in mangaka career, and eventually, in a reprise of the bridge scene, a girl - Airi.
We also know that Airi has an expensive dream... and Satoru, which post-coma ends up as a VERY succesful mangaka (is shown to have a studio with several staffers employed, one says something about celebrating anime adaptation), and will be probably be uniquely suited to help make that dream come true and go with her.
All that said, the change in the hospital scene was sort-of okay, but if they remove that bridge scene, I will rage, hard.

Anyone REALLY don't like how the "mystery" was that the Sensei did it? Literally WHO ELSE could have done it? The story was set up in such a way that it made it stupidly obvious. The killer had to be someone the mother knew and it couldn't be Kayo's mother, Yuuki, or her reporter friend. Those three and sensei were the ONLY adult characters we see in the show, everyone else was a child character and it couldn't have been one of them.

It represents Yashiro attaining heaven (filling the void of his heart) while kicking everyone else down (killing them, cutting off their threads) just like in the Kandatta story. Yashiro was curious as to what could have happened if Kandata arrived to heaven before his thread got cut, in a sense, achieving being complete without having standars for others. But once he gets caught, his thread dissapears since his way of attaining heaven (killing people) got cut off.

If this was the author's goal Satoru should have drowned and died. It would have made sense that that was the cost for saving the lives of all those other people. Maybe if it got really cheesy Kayo could have developed the same powers and went back to save him for real or something, but the way it is now is more just confusingly random than shocking or depressing, considering the kind of characterization we got.
