As far as I'm aware damage done to the user isn't inflicted on the Stand, since something like losing an arm won't damage your soul, but Stands getting damaged have an effect on the user since you're effectively putting your soul out there to get it's shit kicked in. So King Nothing won't lose it's arm but if Damo were to cut of KN's arm both it and Norisuke would lose it. It's the reason why Jotaro had to go after The World instead of DIO, he had no way to effectively kill a vampire at night so he went after his Stand instead since that was his only chance of causing him lasting damage.

Honestly I feel like it's Damo or Joubin. Maybe both, they're antagonising Josuke in different ways for different reasons so they're both main antagonists. There's still the fact that Joubin has bought multiple Rokakaka from Aishou and not one of them has made it's way to Tsurugi and he's keeping it a secret. I feel like once the current arc is over we'll be getting another Joubin arc with a fight that gets a lot more serious than a beetle battle, considering the pacing of JJL so far. I don't see why Damo has to be the muscle, the way he talks and refers to stuff in the flashbacks makes him sound like he isn't taking orders but rather doing stuff of his own accord. I feel like Yotsuyu was meant to be the muscle and the Aphex twins were the enforcers. Damo doesn't seem like ge'd actually make for good muscle considering his Stand isn't as combat oriented as I am a rock. There could be someone above him in the organisation but if they are I don't think they'll be portrayed as the main villain, probably just a side character there to build on Damo's motivations who he kills near the end to take over or something. We'll see though.

It's mostly Damo's stand that makes me sincerely doubt he's the main villain. It's a fantastically terrifying stand but it doesn't seem like a main villain stand, however, it does seem like a Vanilla Ice-like stand. Damo seems like the muscle of whatever the organization is because he's the one who steps in because his lackeys have all been killed.
If anything I think the main villain will have a much deeper connection to either Josuke or the Higashikatas, for me the villain's either going to be Karera because of that ominous quote she said at the end of her last chapter, or the man in the flashback. With how the timeline's arranging so far I'm expecting to see him in the next 2-3 chapters if they're going to be flashbacks.
