It was the lack of creative control in general, his old friend Enokido leaving so he couldn't fufill his original idea for the SuperS movie (they later manifested to form Utena when he went to Enokido after he left Toei), and the terrible working conditions Toei had him in (Anno has said before that Ikuhara would often go for as long as four days without sleeping when he worked on the series.)
He must have really liked Haruka and Michiru, though. Utena herself originally looked exactly like her before they decided to use the design of the girl on the bottom.

That's not what I've heard at all. They always went right alongside the manga. The only time they decided to do a real filler arc was with the Doom Tree. Osabu has said before that they would always spend each November mapping out where the story would go before, so they must have had a general, broad idea of where it would go with very basic plotlines being made. Otherwise, things like Black Lady being in both adaptations could have never happened.

That might be true for the 90s anime, but definitely not for the manga. A big part of the manga was the romance. Takeuchi was always writing a love story. Mamoru was always one of the most important characters. The people behind the 90s anime weren't as fond of Mamoru and really decreased his role sometimes, and it's fine if you like that, but just keep in mind that's not necessarily what the manga/Crystal is about. The 90s anime really changed a lot from the manga, and Crystal is just being faithful to what was originally in the manga. It's important to keep that in mind.

You make it sound like Chibi Moon's lack of offensive powers somehow stopped her from dominating the show. In the manga she had her own Holy Grail to become Super Sailor Moon and it's not like she dominated all the screentime/page time or anything.
I found the fact that in SuperS Chibi Moon had to summon Pegasus every single time Sailor Moon wanted to attack was the stupidest thing ever. It's so inefficient! And when Pegasus is not around, Moon and Chibi Moon are much less useful in a fight, which we see in the SuperS movie and in the battle with Nehellenia's minions in Sailor Stars.
It's pretty telling that the SuperS movie included Chibi Moon's Kaleidomoonscope even though it never appeared in the TV series at all. It's like the writers realized how retarded the setup in SuperS was and said "fuck it".
Also I thought the Starlights' amount of screentime in the manga was fine. They did last longer than almost everybody else before dying. Also their schtick in the anime was unoriginal and the exact same as Uranus and Neptune in S (Oh, we're new Senshi... we're so aloof and edgy... we don't want to work with you.... oh it's almost the end of the season, looks like we have to fight together...), lasting for most of the season. The manga didn't waste our time with this anywhere as much as the anime did.
Also not really related to the Starlights, but i hated the Phages since their lore was so uninteresting and they really only existed to stretch Sailor Stars to fill a whole season. The RoboCop parody was about the only one was I liked.

I remember her saying once that the Stars anime was something she might have done if she had more creative control, so I think she liked most of it outside of the Starlights physically turning male and the lack of Sailor Cosmos.
And yeah, I have no doubt she had some sort of dispute with Toei during that time. What exactly it was we'll never know. Looking back, I'm glad it came back right when it did, since it's return was as powerful as the force of a million dicks ejaculating at once.
One one hand, this looks like it will be much better than the previous seasons. On the other hand, the original anime did this plot arc so well I can't imagine it even approaching it, so it's still redundant.
