There were a few talented guys left. The real blow was that they were fucking Gainax, with a reputation like KyoAni and SHAFT combined. They couldn't help but disappoint everybody.
The only sensible thing would have been to dissolve the studio and start anew (but that would have probably cost them the IPs that Gainax was still getting money for).

I'm not a director, I can't give you a sensible answer as to what would make a GOOD adaptation.
But certain things work in one medium but not the other. Removing things that work in written form but not in animation is a necessity. But on the other hand adding things that work in animation is a good choice.
Change is not bad. The trick is to change things and still maintain the spirit of the original.

I don't get your complaint about the Medaka Box anime then. They added tons of little touches and new scenes, reordered the pre-Unzen stuff to include Kikaijima in more of the story, animated two anime-only episodes written by Nisio, and on and on. The music was solid and the animation, while not anything amazing, stayed consistent during fight scenes. The voice cast was pretty damn good too, even Toyosaki pulled off a performance that went counter to characters she usually did.
