More unsettling than scary tbqh. Don't get me wrong irl if I saw that I'd be fucking terrified.
But in the medium of horror monsters you can't understand have always been the most horrifying. When you can't say it looks like anything else; I think that is a good monster.

I'd say it's more disturbing than scary.
If something is scary it should make you feel paranoid afterwards. Scary is the reason people don't want to walk through the woods late at night.
This is closer to lovecraft type stuff. Horrors beyond our understanding. Still really bloody good, but it doesn't leave me scared of spirals in the same way a good ghost story would scare me off sleeping in an abandoned shack or whatever.

He is one of the twoartists I have seen in my life that can provoke a physical reaction in my through his art.
The only other one was some dead italian guy who made me feel physically disgusted and I had to leave the exhibit. He art was really unsettling, and I couldn't put my finger on why. It was all normal pictures, just his art style was.....ugh.

Not really, his manga are more of the grotesque kind, but they're not really scary. A lot of his works could also be seen as dark comedy, especially the souichi series.
