I always thought of him as someone who struggled to find his own personality and identity? He was hidden, dressed as his sister, made her memories his, then they forced the whole nazi thing on him, etc.. He was a failed experiment and subconsciously didn't cope with it. Also to make it even more obvious, I'll tell you. The "Monster" was not a person, rather you could say its a concept, or situation/feeling that keeps arising in this story in different ways, for different characters.
I think a novel came out afterwards which make it even more clear.

The last chapter is called "The real monster", it keeps trying to get you to question who the real monster was all along, what it was, what it meant. Until Johan's scene in the hospital which reveals some shit.
If I recall, the monster was really something like"The loss of identity"or something similar.

Johan was not the monster in itself, he was just another one effected by the monster. But, that statement is meant to make you question even more what the monster is supposed to be. So many people are called the Monster in the end.

I understood "the monster" as something foreign, unknown and unexplainable to human reasoning.
Similar to the way people's mind tend to think that there is a historical narrative to humans past decisions, when mostly it's just a bunch of random points and guesses.
I think the partial reason to Tenma's shock at the end was because he (and the reader) thought that Johan was the "monster" e' was because some shit happened to him in the past, and then you get the final plot twist that it none of it actually happened to Johan but his sister, making all the previous reasons for his "evilness" moot.

I do agree with your assesment that he's just a kid having fun with toys in a sandbox, but toys might be giving those 'toys' too highly of a care for Johan. He simply sees everyone and everything as ants and messing with the line of ants to see if they come and attack you is always fun.
No one really manipulated him, the more you find out about him, the more you understand that the 'thing that made him evil' was fucked either by him or without him. 511 didn't fuck him, the Rose Mansion didn't fuck him and I don't believe Johan when he says his mother is what made him think everything was evil.
