The most unrealistic thing in this manga though is that no matter how much the bugs have ate through your body, no matter how many vital organs they already ate, somehow you are still able to resist decay and can continue talking even if it is gibberish even though half of the lower part of your body including your stomach is already gone.

It explains how encase you can't read. The maggots eat from the parts which WON'T kill you so that you're kept alive for longer and they excrete a pleasure substance so you feel nothing. It's what bugs actually do except they're sized up and it's applied to humans. Though, this scene is kinda stretching it to an extreme.
