The bitchy act tsunderes put up often is so extreme it can't be seen as anything but their real self, though. The idea of the premise was a series leading the fans of generic harem manga/LNs to believe the plot would go in ways typical to similar series, only to reveal that the girl acting bitchy really is a bitch, and that the girl who one would expect to be rejected and revealed as deeply flawed by the end of the series really was the right girl for the MC. The twist may have been what got elaborated on, but just imagine it's the kind of tsundere character who routinely is aggressive to the MC, but gradually starts liking him. The twist would be that her liking him now doesn't change the fact she's fundamentally an awful person.
