Millenniummon came about when Machinedramon dying after Adventure comes across a Chimairamon and the two merged. Millenniummon was so powerful that like Apocalymon his very existence warped space -time. He defeats all the chosen children and revives Devimon, Etemon Vamdemon and Piedmon Ryo is brought in and defeats him.
But the little asshole won't stay down. In the aftermath of "Or War Game" he comes back and splits the Digital World in two, turning Tai to stone Ryo comes back again and teams up with a young Ken to defeat Millenniummon , also Millenniummon starts htting on Ryo, as in dropping full on sexual innuendo. That's when this happens:

It's also revealed the two are linked, Ryo is the light to Millenniummon's darkness which is why he's the only one who can beat him, but so long as one lives the other will too.
A massive expolsion happens during the fight and the two are flung into the time stream. Ryo ends up at the very start of ever single Digital world in the past well Millenniummon is freed from his cystal becming the even more powerful Zeed Millenniummon and sets up shop in the far future. He then invades every single Digital world in every single time stream heading back to the very beginning* Ryo confronts him one last time and we discover Millenniummon IS Ryo's partner, monodramon forces a jogress with Millenniummon trapping him in the body of Cyberdramon and Ryo decides for reasons unknown to head to the Tamer universe, which he pass by in his adventures in this last game.
*A strange rumor that he released Apocalymon and made Adventure happened is out there, but that is never mentioned in the games.
