Okay, look. The series is obviously not very clear or consistent with the rules of how this shit works, all we can do is piece together whatever bits of information we've been given. Let's look at all the different ways we saw drives happening:
- Extas normally drive by having sex. Usually it looks like they drive the moment they orgasm, but sometimes it looks like it happens before they get there.
- Lady J and Rein can drive without touching each other. Rein explained this happens because "that's how strong their bond is".
- Miranda attempted to force Mamori to drive by giving her an erotic illusion with the prince of her dreams and she seemed quite sure that it was going to work.
- Meifon can drive using money, without interacting with or even thinking about another person. She said that money "sets her body on fire and releases her soul".
- Mamori drove just by looking at Akira's naked body without being touched by anyone. Akira didn't even will it or do anything at all, she just stood there.
So it would seem that for an Exta to drive, she must reach some sort of heightened emotional state of arousal (LadyLady didn't look particularly aroused when they did though, so the arousal may possible be replaced/supplemented by the feeling of a romantic connection or something). The easy way to do this is by having a partner they can focus on and physically interact with, but some Extas have been shown to get there just by thinking about someone or someTHING that gets them hot enough. That's one of the best educated guesses we can make at least.
