Old man inside Ichigo was actually Yhwach from 1000 years ago, apparently present in everyone who inherits his blood (I don't know if it has to be a pure lineage or what)
He was trying to stop Ichigo from becoming a shinigami, but eventually grew to like him and come over on his side to fight against the current-day version of himself.
"Hollow Ichigo" was actually Ichigo's shinigami power from his mother's side, who was infected by a Hybrid made by Aizen. The process of making that Hybrid was similar to how Asauchis are created.
Ichigo has two swords now. Their powers and names are not known.
