real question - what happens when you just move up in weight class but keep your belts? Suppose he moves up to Super Middleweight, but some Jr. Middleweight wants the belt - does Takamura have to drop 2 classes to defend it? Or can he go "fuck you" and refuse the match?

Not likely for a long while. Rising's DVD boxset did poorly in sales and since they went ahead and added the Young Kamogawa arc to Rising, instead of releasing it as an OVA, it's highly unlikely they have any specials planned.
If you're jumping into Ippo from the anime, just start the manga from the beginning. While even the first anime does a good job adapting the source material, there's quite a few difference and cuts made to time-restrictions, especially when you get into the OVAs before New Challenger, much less the laundry lists of changes Rising went through.

With Volg at Jr. Lightweight, we'll likely never see him fight Ricardo. In fact, since Volg is lucky not to be fighting out of Japan, he can still capture the WBO, WBC and WBA world J.Lightweight world titles before the series end.
With as much bullshit he's constantly dumped on him, I'd like to believe him finally winning the IBF title will make it easier for him to have unification matches with other champions at his weightclass, then again, Morikawa loves his drama, so I expect to see some world-class fight-fixing fuckery fuck Volg and Dan over to deny them an easy route of him becoming an undisputed champion.

It's been about 9 years real time since Miyata's had a fight. Given Miyata typical has his major matches before/after Takamura's and EVERYONE has already fought recently, unfortunately it looks like Miyata has a match coming up.
As an aside, I hope Morikawa feels better soon. He already went through a sick spell during Ippo vs Gonzales, and was still pumping out high-quality chapters weekly.
